Aug. 6
1:28 a.m. – A copper-colored mountain bike was stolen from Bobo’s. A report was taken.
6:11 p.m. – Sasha Dee Belden, 43, was arrested for DUII. BAC: .12.
7:09 p.m. – A vehicle reported stolen to the Linn County Sheriff’s Office on July 31 was located near Russell Drive and Primrose Street. LCSO was unable to notify the owner by phone. The residence had a locked gate.

Aug. 5

12:22 a.m. – Peggy Sue Okey, 54, was arrested for DUII. BAC: .23.
12:24 a.m. – Justin Barnett, 29, was arrested on a Lebanon Municipal Court warrant for contempt of court and probation violation, original charge hit and run.
3:31 p.m. – A woman left her cell phone at Walmart at about 1 p.m. She later called her own number and a female answered. She said she would give it back, but the caller has not heard back from her. A report for third-degree theft was taken.
8:34 p.m. – Kimberly Ann Taylor, 32, was arrested on a Klamath County Circuit Court warrant for failure to appear, original charge possession of a controlled substance, methamphetamine.
11:26 p.m. – Samaritan Lebanon Community Hospital staff said a man was hit by a black van while riding his bicycle near Milton and S. Main Streets. The suspect vehicle is a black, possibly 2010, Toyota Highlander or similar SUV. The driver was a white male, 30- to 40-years old. A report for hit and run was taken.

Aug. 4
12:18 p.m. – A caller reported damage to a shed and the box to the sprinkler system, in the 900 block of Weirich Street. A report was taken.
1:11 p.m. – A caller reported a wheel and tire taken off a 2002 Honda Civic overnight, in the 800 block of Walker Road. The tire is a Douglas All-Season. Loss value: $175. A report was taken.
4:31 p.m. – A caller reported his car was hit in the Subway parking lot between 3 p.m. and 4 p.m. A report was taken for hit and run.
5:36 p.m. – A woman found a black Schwinn bicycle in her backyard, in the 600 block of Fifth Street. Estimated value $100. It was entered into LEDS as recovered.
10 p.m. – Craig Steven Bybee, 58, was arrested for harassment and charged with fourth-degree assault. A caller said a man attacked her and her boyfriend, 600 N. Fifth St. Her boyfriend restrained the man but he kept getting loose. A report was taken.
10:21 p.m. – Donna Ashley Lyons, 25, was arrested on a Lebanon Municipal Court warrant for contempt of court, probation violation, original charge second-degree criminal trespass.

Aug. 3
2:16 a.m. – Zachary Wayne Kemper, 20, was arrested for three counts of unauthorized entry into a motor vehicle. A caller said a man was going through cars, in the 300 block of E. Rose Street. He had a backpack and a flashlight. Officers contacted a man at Rose and Cleveland Streets. Kemper taken to the Justice Center. A report was taken.
7:11 a.m. – A purple mountain bike was stolen from the 2600 block of S. Santiam Highway. A report was taken.
9:15 a.m. – A 2001 silver Jeep Grand Cherokee was taken sometime overnight, in the 800 block of W. Rose Street. The vehicle had been towed and did not appear to be damaged. There was a what appeared to be a brand new key the Jeep. An officer attempted to contact the owner. A report was taken.
9:46 a.m. – Joseph Robert Vargas, 33, was arrested on a state-wide felony warrant from the Oregon State Parole Board and a parole violation. Vargas crashed his bike near Rose and Second Streets and ran through backyards. He was detained at 9:50 a.m. across from Linn-Co. A report was taken.
9:14 p.m. – Edward Eugene Allen, 49, was arrested on a Lebanon Municipal Court warrant for contempt of court, probation violation, original charge second-degree criminal trespass.

Aug. 2

12:24 a.m. – Someone tried to break in to La Mota, 620 S Main St. Lebanon Police Officers responded to an audible alarm at the location. The front and back doors were secure. The business did not appear to be entered. A report was taken.
12:50 a.m. – The front driver-side window of a car was broken between 10 p.m. on Aug. 1 and 12:30 a.m. Aug. 2, in the 900 block of Mountain River Drive. A report for second-degree criminal mischief was taken.
8:36 a.m. – A caller said a man was trying to get into the back door of her residence, in the 200 block of N. Main Street. He was bald and wearing a monkey mask. When he took off the mask, he had scars all over his face. He was knocking at the caller’s door for about five minutes. When she opened the door, he stood there with the mask on and didn’t say anything. Eventually he said he was looking for Charlie, cussed out the caller and left. Officers checked the area but were unable to locate the man.
1:24 p.m. – A caller reported a wake board stolen from his boat while his vehicle was parked in the Walmart parking lot. A report for first-degree theft was taken.
2:50 p.m. – A caller reported a hit and run in the last week in the 0-99 block of Park Street. She noticed the back end of her vehicle was damaged and caved in when she returned from vacation. A report was taken.
3:12 p.m. – A caller was repossessing a vehicle in the 1400 block of S. Main Street and saw the trunk was full of what looked like marijuana. A report was taken.
4:02 p.m. – Raymond Gage, 68, was arrested on a Lebanon Municipal Court warrant for possession of alcohol in a city park without a permit.
7:45 p.m. – Seth Floro, 38, was arrested on a Linn County Circuit Court felony warrant for failure to appear, original charge possession of a controlled substance, methamphetamine. He also had warrants for contempt of court, and failure to appear, original charges of first-degree possession of a forged instrument, first-degree forgery, and giving false information.
8:57 p.m. – A man went behind the counter at Jack in the Box and threw something at an employee. He was a 35- to 40-year old white man who was with a couple of other people. The employee said there was nothing physical, the man thought the employee was hitting on his wife. The employee requested officers counsel the man if located. No report.
8:30 a.m. – A vehicle’s window was shot out with marbles last night, in the 900 block of Mountain River Drive. A report for second-degree criminal mischief was taken.
3:48 p.m. – Nickie Allen Carter, Jr., 51, was arrested for DUII.
7:46 p.m. – A caller said she accidentally left her car unlocked overnight and her prescription bottle was stolen, in the 700 block of N. Fifth Street. The car was undamaged and nothing else was missing. The description she gave officers was the same as a bag turned in on a found property report. The officer returned the prescription bottle to the caller. She did not wish to pursue charges or file a report. No additional report was taken.

Aug. 1

4:34 a.m. – A caller reported a suspicious person in the 1800 block of Carroll Street. Deputies provided the person, who appeared to be in crisis, with assistance and transportation. No additional report.
5:06 p.m. – A caller was subject to a computer scam. He had contacted his bank during the phone call with the suspect and was able to terminate the computer connection and phone call with the suspect before any money was taken. He has taken his computer to Staples to have it cleared of viruses.

July 31
10:18 a.m. – A 20-year-old white male with a beard, not wearing shoes, took $12.06 in merchandise from Safeway. A report for third-degree theft was taken.
11:02 a.m. – Tylor James Setzer, 25, was arrested on an Oregon State Parole Board warrant for parole violation.
4:02 p.m. – A red and gold fishing pole was found in the 33000 block of Tennessee Road. A found property report was taken.
4:41 p.m. – Corey Alan Waterman, 37, was arrested on an Albany Municipal Court warrant for failure to appear, original charge second-degree criminal mischief. He also had an Oregon State Parole Board felony warrant and Albany Police Department warrants for failure to appear, original charge unauthorized entry into a motor vehicle and failure to appear, original charge second-degree criminal mischief.
7:18 p.m. – A Burger King employee reported a missing deposit from Saturday night/Sunday morning. Loss value $857.27. Video surveillance was inconclusive. A report was taken.

July 30
12:02 a.m. – Cory Robert Lake, 43, was arrested for menacing, unlawful use of a weapon, two counts of recklessly endangering another and pointing a firearm at another person.
1:59 p.m. – A caller reported mail theft in the 40100 block of Hilltop Drive. No additional report was taken.
9:57 p.m. – A deputy on park patrol found a truck parked in the dog park area, in the 31200 block of First Street. It was unattended and no one was found in the area. The campgrounds were both quiet with only a few campers out. The upper park lots all had cars in them. A deputy contacted four cars that left right away. All were warned for being in a day-use area. The deputy parked under the light of the boat ramp lot and several cars appeared to be coming to the park then left when they saw him.

July 29
1:26 p.m. – A caller reported mail theft, 30300 block of Rancho Road.
1:52 p.m. – A woman said her computer seized up and a window popped up with a phone number to call to fix her computer. She called the number and gave her credit card number and was defrauded $399. A report was taken.
2:43 a.m. – A female hung herself from a tree in the 900 block of Lebanite Drive.
3:01 a.m. – David Sepulveda Callejas, 35, was arrested for DUII. A deputy was dispatched to a crash on Interstate 5 near milepost 233. According to police reports, Callejas was on his way back to Lebanon from the Spirit Mountain Casino. He went off the roadway and crashed into the center cable median. He was not injured. Callejas consented to SFST`s and was arrested for DUII alcohol. He was transported to the Linn County Jail where he gave a breath sample of .12 percent, according to reports.
4:23 p.m. – Two phones were found in the area of Taco Bell. A report was taken.

July 28
10:44 a.m. – A woman said the side of her home and vehicle were spray painted between 10:30 p.m. on July 27 and the time of the call, in the 2100 block of Janna Court.
10:52 a.m. – An 18-month-old fell out of a third-story window, in the 100 block of E. Elmore Street. The child was conscious and crying when medics arrived. The child was diverted to Corvallis Hospital. The incident is under investigation.
11:34 a.m. – A caller said her entire vehicle was spray painted with gold and purple spray paint. It was parked on the side of the road in the 0-99 block of E. Cedar Street. The front hood, windshield and rear trunk were painted with derogatory terms.
6:52 p.m. – Joshua James Warren, 35, was arrested on a Linn County Circuit Court felony warrant for failure to appear, original charge possession of a controlled substance, methamphetamine. He also had an Oregon State Parole Board felony warrant for parole violation.
10:53 p.m. – Laura Lynn Hughes, 41, was arrested on a Lebanon Municipal Court warrant for contempt of court.

July 27
2:26 a.m. – James Leroy Salvador, 21, was arrested on a Linn County Sheriff’s Office warrant for failure to comply, original charge third-degree theft. He also had a state-wide Lane County Parole and Probation felony warrant for parole violation, original charge third-degree robbery, possession of a controlled substance, methamphetamine and first-degree failure to appear.
12:41 p.m. – A woman said her wallet, which contained three social security cards, her ID and her EBT, was taken two days ago. A report was taken.
12:56 p.m. – A Forest River Cargomate trailer was stolen from Santiam Village South mobile home park some time in the last week. It is white with chrome plating around the bottom and wheel wells. There was particle board and a queen mattress set inside the trailer. The trailer was entered into LEDS. The license number is U514044.
2:48 p.m. – A citizen gave 5 mg of oxycodone and acetaminophen, which had been found, to an officer.
3:20 p.m. – Robert Emmett Tackitt, 37, was arrested on a Lebanon Municipal Court warrant for probation violation.
3:55 p.m. – A deputy contacted a driver who stopped alongside the road to get a bee out of the truck in the 37400 block of Highway 226. No additional report was taken.
5:19 p.m. – A Springfield 20-gauge shotgun was found near S. 10th Street and Vaughan Lane. A report was taken.
5:30 p.m. – A caller said his son’s wallet was taken from his son’s backpack while he was at the skate park between 1:30 and 3:30 p.m. It was black with Cheap goods BMX on it. It contained $220 cash and a Subway gift card. Officers reviewed video, which showed multiple people at the park at the time but the video quality was poor. A report for second-degree theft was taken.
7:08 p.m. – A white and gold iPhone 6 was found at Bob Smith Memorial Park. A report for found property was taken.
7:20 p.m. – Daniel Antonio Ibarra, 23, was arrested on Lebanon Municipal Court warrants on contempt of court and probation violation. He also had Linn County Circuit Court warrants for failure to appear, original charge distribution of a controlled substance, heroin and failure to appear, original charge DUII, unlawful possession of marijuana.

July 26
12:34 a.m. – A caller said two little dogs were barking for more than an hour, in the 700 block of Manor Way. The residents were not home. The dogs were in violation of the municipal code. An officer left a card on the door advising them of the problem requesting a call back. The caller did not want the dog owner to be cited, just advised of issue. The dog owner called LPD and was advised of the complaint and city ordinance.
7:14 a.m. – Two car batteries, an old wallet, $12 in cash and a social security card were taken from the backyard of a residence in the 300 block of S. Second Street. A report for two counts of unauthorized entry into a motor vehicle and second-degree theft was taken.
3:30 p.m. – Darrell Gene Mullenix, 43, was arrested for DUII, BAC: .11.
4:42 p.m. – An 8-year-old boy was bitten by a white medium-sized pitbull with brown markings, in the 1300 block of Grant Street. The bite did break the skin. The dog had no collar and was very aggressive. There was no owner in the area. A report was taken.
4:45 p.m. – Gloria Sue Bennington, 50, was arrested on a Linn County Circuit Court warrant for probation violation, original charge ID theft. She also had Lebanon Municipal Court warrants for second-degree criminal trespass and contempt of court and probation violation.

July 25
5:06 a.m. – Adrean Antonio Magana Rangel, 20, was arrested for DUII. Two passengers were cited for minor in possession of alcohol.
7:15 a.m. – Wendy Marie Garrison, 39, was arrested on a Linn County Circuit Court Supervisory Authority warrant for possession of a controlled substance, methamphetamine.
12:51 p.m. – A caller reported her iPhone Plus was stolen from the pirate festival. A report for second-degree theft was taken.
1:33 p.m. – Cassandra Dee Scott, 26, was arrested for second-degree criminal mischief, second-degree theft and aggravated harassment.
2:08 p.m. – Debit cards were found at Walgreens. A report was taken.
6:34 p.m. – An elderly white man took an envelope with $240 off a counter at Walmart and left the store. A report was taken.

July 24
10:02 a.m. – A caller said she was a vendor at the pirate festival and last night her cash box was taken out of her vehicle. Approximately $4,000 in cash in a gray metal cash box was taken from the unsecured vehicle. A report was taken.
12:27 p.m. – Miscellaneous grocery items were stolen from Walmart. Loss value $87.50. A report was taken for third-degree theft.
3:23 p.m. – Thomas Joshua Gantt, 29, had Lebanon Municipal Court warrants for contempt of court and probation violation.
6:56 p.m. – A man stole a Hoover upright vacuum from Walmart on July 11. Loss value: $169.
10:27 p.m. – James Fuller Plymell, 43, was arrested for third-degree assault.

July 23
2:25 p.m. – A 50-pound bag of dog food was taken from Walmart. Loss value $22.98.
9:10 p.m. – A caller said someone was shot with a gun, in the 600 block of B Street. The man who shot the gun was still armed at the time of the call. A second caller said a man came at her husband with a hammer. The Linn County Sheriff’s deputies responded and found Jerry Lynn Chasteen, 58, of Lebanon, had been shot multiple times. Deputies determined a large-caliber handgun was used in a neighborhood dispute between Chasteen and Donald Barry Avritt, 25, of Lebanon. Chasteen was transported to Good Samaritan Hospital in Corvallis for serious injuries and was listed in stable condition. Avritt and witnesses were detained. LCSO was assisted by Lebanon Fire Department and Lebanon and Albany Police Departments. No arrests have been made. The investigation is continuing into the possibility of this being a criminal matter or self-defense. All parties were cooperating with investigators.

July 22
9:54 a.m. – An employee at a gas station in the 2100 block of S. Santiam Highway received a fraudulent $20. The suspect vehicle was an older Dodge Stratus sedan. The driver was described as an older white male with a beard. Other people were in the vehicle. Viewing of surveillance video was pending. The counterfeit bill was seized. A report was taken.
10:06 a.m.- Anjanette Ray Baker, 65, was arrested on a Lebanon Municipal Court warrant for contemt of court and probation violation.
10:38 p.m. A black Ford Expedition ctrashed into a house in the 2700 block of S. Main Road. The roof on the residence fell in. The caller was not injured but could not get out of her house. Medics transported the driver, Brianna Marie Housworth, 20, who was cited for minor in possession of alcohol and reckless driving. A report was taken.
11:34 p.m. – Daniel Austin Pope, 24, was arrested on a Linn County Circuit Court warrant for failure to comply, charges two counts of unlawful use of a weapon, three counts of strangulation, two counts of menacing and fourth-degree assault.

July 21
12:22 p.m. – A silver men’s mountain bike was leaning against a telephone pole near the Lebanon Community School District Office. It was entered into LEDS as recovered property.
1:50 p.m. – Jennifer Ann Martin, 40, was arrested on a Linn County Warrant for failure to comply, original charge second-degree theft.
1:51 p.m. – Matthew Alexander Taylor, 25, was arrested on a Lebanon Municipal Court warrant for contempt and probation violation.

July 20
9:04 a.m. – Jacob Noel Yonker, 32, was arrested on a Linn County state-wide felony warrant for probation violation, original charge, possession of a controlled substance, methamphetamine.
9:31 a.m. – A caller reported her unlocked vehicle was entered overnight, in the 2600 block of S. Main Road. An older Pioneer brand surround sound system and the battery from under the hood of the car were taken.
9:50 a.m. – A caller reported a 2004 gray Chevy pickup was stolen while parked on 11th Street near the U-Haul. It was entered into LEDS/NCIC
11:16 a.m. – A caller said his neighbor’s dogs run loose outside every day and “mess” in the caller’s yard, in the 1100 block of Hiatt Street. When an officer arrived two medium-sized black dogs were running at large. A resident, who was not the dog owner, helped the officer get the dogs inside the house. A report was taken.
2:27 p.m. – A vehicle was entered overnight, in the 100 block of Dodge Street. A multi-colored pocket knife was taken. There also was a gash in the side wall of one of the tires. Loss value $150. A report was taken.
4:25 p.m. – Aaron Martin Heintzman, 28, was arrested on a Lebanon Municipal Court warrant for contempt of court, original charge driving while suspended and probation violation.

July 19
8:03 a.m. – A caller said someone used bolt cutters to open a cabinet at River Park and stole a roll of toilet paper. A report for third-degree theft was taken.
8:37 a.m. – A caller said his residence was entered while he was sleeping, in the 300 block of Park Street. His computer, PlayStation 3, and a makeup bag were taken. He believes he left the door unlocked. A report for first-degree burglary was taken.
9:50 a.m. – The exterior of the basketball court and gym were tagged with spraypaint overnight. A report was taken.
12:06 p.m. – An Aaron’s Electronics employee reported that a man took a silver 10-inch HP X2 detachable laptop. The suspect is a white male, approximately 20 years old, 5 foot 7 and very skinny. He was last seen wearing a white shirt and khaki shorts. Aaron’s valued the laptop at $506.99. The serial number is 5CD610RX0. A report was taken.
2:27 p.m. – Daniel Dee Walburn, 33, was arrested on on a Linn County Circuit Court warrant for failure to appear, original charge possession of a controlled substance, methamphetamine and failure to report as a sex offender.

July 18
4:53 p.m. – A caller said a male just broke into his vehicle which was parked at the Veterans’ Home. Another vehicle was reported broken into as well. The vehicles were unlocked and parked next to each other. Both owners wanted the incident documented only. The suspect was described as a young white male, wearing a black hoodie and cargo shorts and was riding a bicycle. He fled eastbound when staff exited the building. No additional report was taken.
5:57 p.m. – A caller reported someone broke the passenger window of her vehicle sometime overnight, in the 0-99 block of W. Grant Street. A report was taken for unauthorized entry into a motor vehicle, second-degree criminal mischief and third-degree theft.
7:13 a.m. – A caller said a package of medical supplies was delivered to her residence on July 11 but she never received the package. She believed the package was stolen from her porch. Her insurance company provided new supplies at not extra charge. A report for mail theft was taken.
9:44 a.m. – A caller reported a bag was taken from his unlocked vehicle overnight, in the 200 block of S. Third Street. The black leather bag contained various medical paperwork from Bridgecreek and the Amber Care facilities. Nothing else was missing. A report was taken.
10:58 a.m. – Thomas Francis Chohan, 45, was arrested on a Lebanon Municipal Court warrant for contempt of court and probation violation.
12:37 p.m. – Robert Edward Hillard, 48, was arrested on a Linn County Justice Court warrant for contempt of court.
2:02 p.m. – A caller said he was tricked into giving someone $500 via a Green Dot card over the phone. Report to be forwarded to the Department of Justice.
7:52 p.m. – A caller reported some items were stolen from the work box and cab of a truck at Best Western Boulder Falls Inn. The man came out of the hotel and noticed some of his things on the ground next to his truck, which was parked in the gravel lot across from the hotel. Stolen items include a black Flir thermal camera, valued at $700 and a Bosch laser measuring device, valued at $150. the spare keys were taken as well. A report was taken.

July 17
8:07 a.m. – The glass on the Lebanon High School commons area roof was spray painted over the weekend. The damage estimate was more than $1,000.
11:20 a.m. – A caller reported a homeless camp with a fire at the old mill site, 800 Milton Street. The fire was not out of control. Four men were trespassed.
12:53 p.m. – A hay truck damaged a signal at S. Main and Wheeler Streets. LPD contacted the Oregon Department of Transportation for a hit and run.

July 16
12:24 p.m. – A caller reported four-wheeler tracks through the field from the racetrack to his property. Four Alcoa brand aluminum wheels were taken. Value $1,000.
5:46 p.m. – The shed at the soccer fields was broken into sometime between Friday at 5 p.m. and the time of the call. A large blue ice chest with “AYSO” in black type on the top, two 330-foot plastic tapes with “AYSO” on them and a white 5-gallon bucket containing hammers, spikes, string and a scoop shovel were taken.
6:35 p.m. – A black mountain bike was taken from a garage, in the 2300 block of S. Fifth Street. A report for third-degree theft was taken.
7:06 p.m. – Robert Wayne Brown, 72, was arrested for DUII. BAC .268.

July 15
12:18 a.m. – A Samaritan Lebanon Community Hospital staff member requested an officer keep a patient’s firearm as it could not be transported with him to the Albany hospital and it could not be kept at SLCH. It was seized for safekeeping.
9:39 p.m. – Bryson Setzkon, 28, was arrested on an Albany Municipal Court warrant for failure to appear.

July 14
3:41 a.m. – Kevin Lee Ashe, 25, was arrested on a Lebanon Municipal Court warrant for contempt of court and probation violation, original charges third-degree theft and second-degree theft.
8:02 a.m. – A caller said his trailer was spray painted overnight, in the 2400 block of S. Sixth Street. His neighbor’s truck and Cascades School also were spray painted. The principal said she would review surveillance video and follow up with officers. A report for second-degree criminal mischief.
1:49 p.m. – A white adult male took a gold X-Box controller, value $159.84, from Walmart. It was entered into LEDS.
7:14 p.m. – Brandy Ln Parker, 36, was arrested on a Lebanon Municipal Court warrant for contempt of court and probation violation.

July 13
8:12 a.m. – Rikkiah May Alexander, 29, was arrested on a felony warrant out of Linn County Circuit Court for probation violation, original charge distribution of a controlled substance, methamphetamine. Zachary Lorain, 29, was arrested on a Lebanon Municipal Court warrant for contempt of court, original charge third-degree theft. He also had a Linn County Circuit Court warrant for probation violation, original charge fourth-degree assault and an Albany Municipal Court warrant for failure to appear, original charge second-degree theft.
9:36 a.m. – A caller reported a trout fishing pole, reel and tool kit were taken from an unlocked vehicle sometime after 5 p.m. July 12 and 8:30 a.m. the day of the call, in the 200 block of Dodge Street.
5:59 p.m. – Steven Andrew Williamson, 49, was arrested on a Sweet Home Police Department warrant for third-degree theft.

July 12
12:05 a.m. – Aaron Michael Barger, 32, was arrested on a Lebanon Municipal Court warrant for contempt of court and probation violation. He also was charged with possession of a controlled substance, methamphetamine.
5:15 a.m. – A caller reported his Taurus 38 special revolver taken from the center console of his unlocked vehicle overnight, in the 200 block of S. Williams Street. His vehicle was parked in the driveway behind his residence. A report was taken and the gun was entered into LEDS/NCIC.
7:22 a.m. – A caller’s purse and wallet were taken from her unlocked vehicle overnight, in the 400 block of Wheeler. The brown leather Coach purse was valued at $300. There was $20 cash in the wallet. A report was taken.
8:18 a.m. – A backpack was taken from a vehicle behind Schmizza Public House sometime between 5 p.m. and 11 p.m. on July 11. There was approximately $200 worth of school books in the missing backpack. The caller believed there may be surveillance video of the area. A report was taken.
8:37 a.m. – Two vehicles were broken into sometime overnight, in the 0-99 block of Park Street. Two pairs of shoes, blue plaid shorts, red plaid underwear, $5 in change and two vehicle headlight lenses/bulbs were stolen. A report was taken.
11:36 a.m. – A caller reported a backpack containing an iPad was taken from his vehicle, in the 0-99 block of W. Sherman Street. The iPad was entered into LEDS.
11:38 a.m. – Samaritan Lebanon Community Hospital emergency room staff reported a combative patient. The patient bit a nurse. The patient was issued a citation for fourth-degree assault. A report was taken.
6:15 p.m. – Jerrold Bryan Campbell, 65, was arrested for DUII and reckless driving. Campbell crashed into another vehicle when he pulled out of Lebanon Plaza. He then fell and hit his head. He was cleared from Samaritan Lebanon Community Hospital. Campbell consented to a breath test. BAC: .22.
